The judges commented, “we really appreciated the whole team approach and embracing the skill mix of the Team, for example, the fantastic work by your care-coordinators; development of robust processes such as your protocols and innovatively developing your Daffodil Line. We also really liked that you focused on collecting and learning from feedback from people with lived experience and the comments you received are heart-warming. You have clearly demonstrated your Practice’s commitment to achieving meaningful and consistent quality improvement for patients and families affected by advanced serious illness, end of life care and bereavement”.

The formal presentation of the award was held at the RCGP annual conference on 19 October 2023.
